By registering for the Epilepsy Foundation Northwest Northwest Run/Walk for Epilepsy, I hereby release Epilepsy Foundation Northwest, the City of Portland, Portland Parks and Recreation, Portland Rose Festival Foundation, and all other sponsors, volunteers, and organizers from any and all liability arising from illness or injury I may suffer at Northwest Run/Walk for Epilepsy. I further authorize Epilepsy Foundation Northwest the right to use my image or likeness for the purposes of display, publication or advertising. I understand that I shall receive no compensation for my appearance. I waive any right that I may have to approve the images used in connection therewith.
